| Birth Certificate (Authenticated by the Philippine Statistics Authority)
| Marriage License (valid for 120 days)
  or Marriage Contract (valid for 120 days)
  or Affidavit of Cohabitation (for those who lived together as husband and
       wife for at least five (5) years)
| Two (2) copies of recent 2x2 pictures
| List of Sponsors and their addresses (Maximum of 8 pairs)
| Ecclesiastical Banns and Permissions (Valid of 6 months)

Additional Requirements for those WITH A PREVIOUS MARRIAGE
| Authenticated Death Certificate of the deceased partner (for widower/ widow)
| Document of Nullity from the competent Matrimonial Tribunal (for annulled marriages)

Additional Requirements for a MARRIAGE WITH A FOREIGNER
| Certificate of Freedom to Marry from foreigner's consulate/ embassy
| If Japanese, a clearance from the Office of Fr. Toru Nishimoto

Additional Requirements for a MARRIAGE BETWEEN
A CATHOLIC AND NON-CATHOLIC
| Dedication/ Baptismal Certificate of the Non-Catholic Party
| Certificate of Freedom to Marry from the Non-Catholic Minister of Pastor
| Dispensation from mixed religion or disparitas cultus, as the case may be
